The name that appears in your record is the official legal name under which a student was admitted. If you wish to change your name, you must provide the Registrar’s Office with supporting documentation of your new legal name. A Change of Name form can be found in the CAU Website. Support documentation: includes marriage certificate, Social Security card, passport, driver’s license or court order. Once we receive and verify the documentation, the Registrar’s Office will proceed to change the student’s name in his/her official academic record. The Registrar will keep a record of the former name. At the discretion of the Registrar, minor changes (e.g., spelling, corrections) may be made; the student may be required to provide documentation to support the change. A fee will apply.
