All diploma replacement requests must be submitted in writing. The request must indicate the address that it should be sent to and should bear your signature. A fee of $40 is required. A notarized letter explaining what happened to the diploma (ex: lost) has to be included. The letter needs to include your current name, name as seen on diploma (if different), signature, student number, degree, major, term, and year of diploma. It will take approximately 60 days for you to receive your new diploma. You can mail your request to the Registrar’s Office.
